定义:早产儿动脉导管未闭(PDA

健康新生儿中,在出生后的前72小时内,动脉导管未闭并不是病理性的,在这段时间内,99.5%的导管会在这个时间段内自发关闭。然而,与此相反,在出生体重<1000 克的早产儿中,大约有 1/3 早产儿会在生后的第2-6天发生自发关闭(越不成熟的早产儿,自发关闭的就越少)。

发病机制和临床意义

动脉导管的开放所引起的问题包括:肺再循环/高灌注、体循环舒张期低灌注以及左心室收缩期容量超负荷而导致的心脏负担。
流行病学研究表明,PDA与早产儿以下并发症有关:
· 死亡率增加[1]
· 脑出血和 PVL [2, 3]
· 支气管肺发育不良 [4, 5]
· 肺出血[6]

· 坏死性小肠结肠炎[7]

PDA的预防性处理:

预防处理:
  1. 药物预防

理由

小早产儿中PDA的发生率高以及与之相关的高发病率和高死亡率表明,提示通过预防性干预措施(如布洛芬、吲哚美辛或手术结扎)可降低PDA相关的并发症发生率和死亡率可能是合理的。

证据

布洛芬与安慰剂或对照组:荟萃分析( Ohlsson & Shah Cochrane 2011 [8]

 

防性给予布洛芬降低了生后3 PDA的发生率(RR:0.36;95%CI:0.29-0.46NNT 4),并降低了需要手术结扎的PDA的发生率(RR:0.495%CI0.18-0.88NNT 33)。

但是,研究并未发现布洛芬对出院时的死亡率(RR:0.9095%CI:0.62-1.30)、1-4IVH (RR:1.0095%CI:0.82-1.23), 3-4IVH (RR:0.82; 95%CI0.54-1.26), PVL (RR1.19; 95%CI:0.64-2.18)、BPDRR:1.04;95%CI:0.87-1.25)、ROPRR:1.02;95%CI:0.73-1.41)、NECRR:1.0495%CI:0.63-1.70)或达全肠道喂养时间(0.47d-2.99 - 3.94)、住院时间(差异为-2d-7-4)、利尿剂(差异为-0.05mL/kg/h-0.26-0.15)等方面有获益。

另一方面,口服布洛芬后出现胃肠道出血增加(RR:1.99;95%CI:1.13-3.50,并且,在第3天肌酐水平上升(差异为0.09mg/dl; 95%CI:0.05- 0.13md/dl),过值得注意的是,相关研究的结果存在显著异质性(I²=65%)。

吲哚美辛与对照组:FowlieDavisMcGuire Cochrane 2010 [9]

 

预防性给予吲哚美辛降低了 PDA 发生率[所有患者:(RR:0.29; 95%CI:0.22-0.38) ,仅在症状明显的患者:(RR:0.44; 95%CI:0.38-0.50; NNT 4)],并减少了需要手术结扎的PDARR:0.5195%CI:0.37-0.71NNT 20,此外,还降低了1-4IVH (RR:0.88; 95%CI:0.80-0.98; NNT 25), 3-4IVH (RR:0.66; 95%CI:0.53-0.82; NNT 20)、脑室扩大、PVL、脑白质回声异常(RR:0.80;95%CI:0.65-0.97;NNT 25)的发生率。

 

然而,尽管如此,在存活率或神经系统预后方面并未证明有显著优势(RR:1.02; 95%CI:0.90-1.15)。此外,IVH 的进展(RR:0.80; 95%CI:0.44-1.47)、CLDRR:1.06;95%CI:0.92-1.22)、NECRR:1.09;95%CI:0.82-1.46)、ROP>3RR:1.75;95%CI:0.92-3.34)、肺出血(RR:0.84;95%CI:0.66-1.07)、血小板减少(RR:0.50; 95%CI:0.11-2.22)、机械通气时间(-1.83;95%CI:-5.53-1.87)以及胃肠道穿孔(1.1395%CI:-5.53-1.87)等方面,未见明显差异。

但更常见的是发生少尿/无尿(RR:1.90; 95%CI:1.45-2.47,但没有发现肌酐水平升高(RR:1.09; 95%CI:0.47-2.51)。

证据

预防性手术关闭导管与不干预组比较  Cassedy et.alRCT  1989 [7]

结果显示,在出生后第一年的死亡率 (RR:1.06; 95%CI:0.75-1.49), 3-4IVH(RR:0.81;95%CI:0.52-1.28), BPD (RR:1.07; 95%CI:0.68-1.69) ROP>3 (RR:0.32;95%CI:0.04-2.80)等方面没有优势,但 3期以上NEC (RR:0.25; 95%CI:0.68-1.69) 显著降低,干预组和对照组的 NEC 发生率分别为 8% (3/40) 30% (13/44)

 


结论和建议:

所有研究中报告的 PDA 结扎率都超过了图宾根近年来观察到的 PDA 结扎率数倍之多。

TIPP 研究中,减少PVL(脑室周围白质软化)/ IVH(脑室内出血)与两岁时对NDI(神经发育障碍)没有影响之间存在未解决的矛盾。

 

由于50%PDA患者在没有预防性干预的情况下也会在最初几天内自然闭合,这种预防性干预会使他们面临不必要的风险,且没有证据表明通过干预可以改善长期预后或降低死亡率。所以我们的建议是在严重脑出血的发生率比较低的情况下,不建议采取预防性干预措施。

 


PDA的治疗措施:

PDA的诊断:

PDA的临床表现是:收缩期或收缩-舒张期机器样杂音,通常位于左侧胸骨旁第 23肋间,搏动强,心前区搏动增强,脉压大,动脉压低伴有舒张压非常低,肺功能/气体交换恶化、少尿和易水肿,有时喂养不耐受。通过超声心动图比临床检查或放射学检查更早确诊 [10-13]

药物治疗:

  1. 1.  指征

证据

目前已有关于早期治疗 "显著 "PDA [Mitra et alCochrane 2020]和治疗症状性PDA[Evans et alCochrane 2021] 的荟萃分析,但这些分析结果,都未显示出足够证据表明治疗PDA 能改善发病率或死亡率。

这些荟萃分析中缺乏足够的数据对治疗与不治疗进行比较。所有纳入的研究都存在 "污染 "问题,即在本来不治疗组使用开放标签治疗PDA[Hundscheid BMC Peds 2018],因此无法对治疗与不治疗进行比较,也就无法确认导管闭合的有效性,以及其可能对降低早产儿并发症和预后改善的作用。

在本指南的本次更新版本中,应明确提及近期最新的研究:

BENE-Ductus 研究[Hundscheid et alNEJM 2022] 针对胎龄<28周且PDA>1.5mm 早产儿的多中心非劣效性 RCT研究。

不足之处:非盲法,并且由于招募不足,只纳入了273例,而计划招募564 例早产儿。

优点:与我们有关的是需要治疗的导管的定义以及拯救性疗法,几乎没有污染(对照组中的患儿确实未因PDA接受治疗)。

果:在生后24-72h内,与接受治疗的PDA组相比,期待治疗不劣于治疗组。在接受 PDA 治疗组中,BPD有明显增加的趋势:期待治疗组BPD发生率为39/11733.3%),而治疗组BPD发生率为57/11250.9%)(绝对风险差异,-17.6%;双侧95% CI-30.2 -5.0)。

Baby-Oscar研究(仍未发表,只有大会论文)

对胎龄<29 早产儿且 PDA>1.5mm 的多中心优势性RCT研究。

该研究的优势:采用了双盲、安慰剂对照。不足之处:相当多比例的开放标签PDA治疗(在 "安慰剂组"中,药物治疗占30%,结扎占10%),"几乎 "达到预定的病例数(计划病例数为730,实际病例数为653)。

结果:没有证据表明生后72小时内且PDA>1.5mm且有持续动脉导管血流的早产儿给予布洛芬治疗的优势,在主要结局及临床相关的次要结局方面均无明显优势。对风险差异的估计更有利的倾向于安慰剂组。(这并不是因为病例数不足)。

由于缺乏证据证明(早期)关闭导管治疗具有长期益处及现有治疗可能的副作用风险,与既往发表的临床研究相比,对COX抑制剂的治疗采用更加严格的使用似乎是合理的。在NICHD试验完成并发表后,需要进行新的评估。

 

推荐

如果临床上怀疑有持续的 PDA,应始终进行超声心动图检查。超声心动图检查应主要由新生儿科团队进行。

如果符合以下标准,即为使用药物进行导管关闭治疗的临床指征:

a.) 肺出血(气管中有新鲜血液分泌物)并证实存在以左向右分流为主的 大的”PDA(体重<1000 g的早产儿二维图像中最窄处的PDA直径≥ 1.5 mm,体重较大的早产儿≥1.5 mm/kg)的依据

(虽然肺出血和 PDA之间似乎存在相关性,但也有小的PDA 或无 PDA的情况下也会有肺出血的情况,因此似乎没有必要主动干预治疗)。

或者

b) BENEDuctus BabyOSCAR临床试验中的拯救性处理标准

至少符合以下至少2项判定标准定义为大PDA

1.     2D成像中最狭窄处的”PDA直径在1000g以下的早产儿中≥ 1.5mm,或在更大的早产儿中≥ 1.5mm/kg(根据1996Evans2022Hundscheid的图宾根版本);

2.     在持续左向有右分流的情况下,舒张期动脉导管处流速缓慢,血流流速小于1/秒(也就是"无压力差"情况)(注意:如果 PDA 非常小,测得的舒张期流速也可能较低);

3.     左心房与主动脉比值(LA: Ao> 2.0

4.     右或左肺动脉舒张末期流速>0.3 m/s

至少满足以下一项超声心动图的标准用于拯救性治疗的临床指征

1.     显的左心室容量负荷导致二尖瓣不全;

2.     明显的全身性灌注不足导致大脑前动脉(ACA或降主动脉内舒张末期返流;

3.     大的PDA伴随的肺动脉高压迹象,经确认并非由严重疾病(胎粪吸入、肺发育不全、败血症.)引起的。

或者

c.) 肺部疾病(作为肺部高灌注的潜在征兆),定义为从出生后第 2 周起通过一下任一标准确定:

1.     需要机械通气

2.     CPAP支持且需要额外氧气(最少浓度为30%

并且至少满足下列超声心动图标准中的4,即满足必须要治疗的动脉导管的情况

1.     大的PDA的定义为:在体重<1000g的早产儿中,在2D超声成像中采集最狭窄处的PDA直径≥ 1.5mm;体重>1000g的早产儿,最狭窄处的PDA直径≥ 1.5mm/kg[Evans 1996 Hundscheid 2022 的图宾根修改版本]

2.     在持续左向右分流的PDA,舒张期动脉导管处流速缓慢,血流流速小于1/秒(也就是"无压力差"情况)(注意:如果 PDA 非常小,测得的舒张期流速也可能较低);

3.     大脑前动脉(ACA)或降主动脉内舒张末期返流;

4.     明显的左心室容量负荷导致二尖瓣反流;

5.     大的PDA伴随的肺动脉高压迹象,经确认并非由严重疾病(胎粪吸入、肺发育不全、败血症)引起的;

6.     LAAo>2.0

7.  右或左肺动脉舒张末期流速>0.3 /秒;

  1. 2.     选择COX抑制剂及其使用剂量

证据

PDA闭合率方面,吲哚美辛和布洛芬之间没有显示出差异。

布洛芬静脉给药和口服给药在PDA闭合率方面也没有发现差异。

鉴于只有布洛芬的静脉注射应用得到批准,并且观察到布洛芬的短期肾脏副作用较低,因此,我们推荐将布洛芬作为一线治疗药物[14, 15]

在分析的研究中,布洛芬的使用剂量如下:初始剂量10mg/kg/d,随后每 24小时两次,每次 5mg/kg/d

推荐

布洛芬静脉注射(初始剂量10mg/kg,随后24小时两剂,每剂5mg/kg

在必要的情况下,如果需要进行第二次治疗周期,可以从第5天开始将剂量增加2倍(个体化决定、前提条件是尿量>1ml/kg/h)。如果在第 2 周开始治疗时:每24小时一次,推荐使用剂量20/10/10 mg/kg(注意:对于IUGR/明显的低出体重/产前因肾脏问题导致的羊水少或产后少尿等情况则应减少剂量)。

 

如果大剂量布洛芬治疗失败并且仍然需要闭合PDA则使用吲哚美辛

单剂吲哚美辛 0.2 mg/kg 0.4mg/kg,原则上每 8 小时一次,使用常规静脉给药,如果无其他静脉通路的指征,也可以肌肉注射。

 

在尝试用药物闭合动脉导管后,如果临床指征需要进行持续处理,即需要下一次给药时,应进行一次结果/病情进展评估。(即最后一次使用布洛芬约24小时后或最后一次使用吲哚美辛约8小时后)。

 

使用 COX 抑制剂的禁忌症:

绝对禁忌:管依赖性心脏病、肾功能衰竭或少尿(利尿 <1ml/kg/h,活动性出血(肺出血除外)、未经治疗的高胆红素血症、急性NEC

相对禁忌:血小板减少(相对禁忌<50,000/µl)、少尿<2ml/kg /h

如果有禁忌症,可进行结扎/介入性导管关闭措施或尝试使用对乙酰氨基酚进行治疗(个体化决定)。

由于结扎与神经系统不良预后、BPDROP 和脊柱侧弯等疾病密切相关[19],结扎手术只能作为药物治疗导管闭合失败或存在药物治疗的禁忌症,且存在紧急临床治疗指征的情况下的最后手段(个体化决定)


患儿的其他药物/状态对治疗结果的影响:

  1. 1. 在使用COX-抑制剂治疗期间使用呋塞米

证据

呋塞米对使用吲哚美辛治疗期间早产儿影响的荟萃分析[20]3项研究共70名患者[21-23]。对PDA关闭率无明显影响,但这些研究均未考虑如IVHBPDNEC或神经系统预后。

此外,较新的研究[24]:单中心、前瞻性、RCT研究,共纳入67例体重<2000g、孕周<34周的早产儿,在出生后96小时内使用吲哚美辛,干预措施:1mg/kg静脉输液呋塞米组与安慰剂组比较;

主要结果:PDA关闭;该研究因为呋塞米组急性肾功能不全发生率增加而提前终止

意向治疗分析:PDA关闭率无差异(29/34 vs 27/29;p=0.437),但急性肾功能衰竭(血清肌酐>1.6mg/分升)在呋塞米组(20/34)远高于安慰剂组(3/29)(p=0.001);

在胎龄<29周的亚组分析:呋塞米组(10/1953%)与安慰剂组(3/1718%)相比吲哚美辛剂量更多,手术结扎治疗更多(OR 5.1995% CI 1.11-24.14P = 0.041)。

推荐

在使用COX抑制剂治疗时候,尽量避免使用呋塞米



  1. 1. 减少液体摄入量

理由

减少液体摄入量可以通过减少血管内液体来减轻由PDA左向右分流(LRS)引起的肺充血和心脏负担的后果。

证据

1项荟萃分析[25]有下列结果:

PDA 4项研究,526名患者:0.52[0.370.73] NNT 795% CI 5-14

一项纳入了60名胎龄<30周且体重<1000g的早产儿的前瞻性随机单中心研究表明,在总液体摄入量相同的情况下,给药方式(部分肠内与完全肠外)对 PDA 发生率有影响(PDA发生率:36% vs 69%RR:0.5295% CI0.30; 0.90; P=0.02) [26].

随后进行的一项两中心前瞻性随机研究纳入了214 胎龄<31周并且出生体重 >400g<1000g的早产儿,该研究未能证实这一观察结果(PDA 治疗 48% 55% RR 1.16; 95% CI0.89-1.50) [27].

推荐

如果超声心动图诊断出具有血流动力学意义的PDA(定义:持续的左向右分流的PDA且符合下列标准的其中两项:LA/Ao>1.4PDA 直径>1.4mm大脑前动脉阻力指数>0.9或在腹腔干处血流为 0,总液体摄入量的等热量降低15-20%(即每天每千克体重130ml代替150ml;每天每千克体重150ml代替180ml



实际应用:

1. 预防

·   不进行药物或手术PDA预防干预治疗

 

心脏杂音和以下情况的超声心动图检查诊断PDA或可能需要治疗的PDA:

·   肺出血

·   胎龄<28周早产儿,生后2周仍持续需要机械通气/氧需求升高FiO2>0.3

·   循环功能不足(特别是舒张压较低时)或者其他导管未闭引起的临床征象(参见下方)

如果超声心动图的迹象提示持续存在并且PDA仍然开放但(还)不需要治疗,那么超声心动图检查通常最多每周进行一次。

2.    PDA治疗的指征:

a)    肺出血(气道有新鲜血液分泌物)和证实存在左向右为主的大的”PDA<1000g早产儿最狭窄处的直径于二维图像上显示³1.5mm,较大早产儿³1.5mm/kg

或者

b) 符合在 BENEDuctus BabyOSCAR临床试验中的拯救性处理标准(图宾根修改版本)

至少满足以下两个超声心动图大PDA的标准:

1.     " "PDA定义,体重<1000g的早产儿,超声2D成像显示PDA最窄处的直径≥ 1.5mm或体重较大的早产儿≥ 1.5mm/千克 [Evans 1996 Hundscheid 2022 的图宾根修改版本]

2.     搏动性/非限制性血流PDA(在持续的左向右分流情况下,舒张期动脉导管处流速缓慢,血流流速小于1/秒(也就是"无压力差"情况))。(注意:如果 PDA 非常小,测得的舒张期流速也可能较低)

3.     LAAo>2.0[图病根修改版本]

4.     右或左肺动脉舒张末期流速>0.3/

并且至少满足以下一项作为拯救性治疗的临床指征

1.     明显的左心室容量负荷导致二尖瓣反流。

2.     明显的全身性灌注不足导致大脑前动脉或降主动脉内舒张末期返流。

3.     大的PDA伴随的肺动脉高压迹象,经确认并非由严重疾病(胎粪吸入、肺发育不全、败血症…..)引起的[图病根修改版本]

或者

c.) 肺部疾病(作为肺部高灌注的潜在征兆),定义为从出生后第 2 周起通过一下任一标准确定:

1.     需要机械通气

2.     CPAP支持且需要额外氧气(最少浓度为30%

3.    开展PDA治疗

·      药物治疗

首选药物(因为获得批准)

布洛芬静脉注射,在出生后第一周的初始治疗中使用剂量10/5/5 mg/kg/次,每 24小时一次(如有必要进行第二轮治疗,可将剂量增加到 20/10/10mg/kg/次,每 24小时一次)

次选药物(当布洛芬无效时)

吲哚美辛静脉注射或肌肉注射 0.2mg/kg/次,每8小时

(在没有效果显现时,24小时后提高剂量到0.3-0.4mg/kg.次,每8小时)

绝对禁忌:导管依赖性心脏病,急性NEC,急性2级以上IVH,或者急性出血(除肺出血外),同时进行的糖皮质激素治疗,少尿/无尿或严重肾功能不全。

相对禁忌:血小板减少(<50.000/µl

光疗时胆红素升高

同时接受氨基糖苷类药物治疗 氨基糖苷类药物剂量减少1/3

 

在尝试用药物闭合动脉导管后,如果临床指征评估需要进行持续处理,即需要下一次给药时,应进行一次结果/进展评估。 (即最后一次使用布洛芬约24 小时后或最后一次使用吲哚美辛约 8 小时后)。

通过增加剂量进行治疗,使得通过导管的血流至少达到限制性血流模式/"闭合模式"(通过导管的血流舒张末期流速>1.5m/s)。

(在治疗结束后一周左右再次监测收缩是否持续)。

 

对乙酰氨基酚尝试治疗:个体化决定

手术或介入治疗:      个体化决定

支持性措施:(PDA满足至少1个上述超声心动图标准时实施):

PDA情况下,将新生儿的总液体摄入量减少15-20%,同时保证摄入的热卡不变)(原本每天每千克体重应摄入150ml,那么在PDA情况下应减少到每天每千克体重130ml;如果原本是180ml,则减少到150ml),选择稍高的PEEP、允许高碳酸血症。
